Clinical Observation on the Fractures of the Forearm Bones


Abstract
Clinical analysis was made on 62 cases of fractures of forearm bones which were treated by conservative method, 30 cases, open reduction 32 cases, at the department of Orthopedic Surgery, Ewha University, College of Medicine, Seoul,
during the Period from April 1972 to August 1977.
Generally, fracture of forearm bones is a difficult treatment due to its special anatomical function and activity, such as supination, pronation, interosseous space, and maintain of its bone length.
The cases of injury were mostly falling, stumbling, sports -trauma (24cases; 38.7%), traffic accident (19 cases; 30.7%), next, wringer injuries with twisting and compressing. (9 cases; 14.5%).
In fractures of forearm bones, most common site of the fracture was distal (44 cases), middle (26 cases), Proximal (20 cases), especially Montaggia fracture (5 cases).
Average healing time of close reduction group was radius fracture only 4 weeks, ulnar 5 weeks, both bone fractures 8 weeks, and wringer injury with open fracture 9 cases 13 weeks.
The author performed open reduction in 32 cases, using round Steinmann Pin 18, general plate and screw 27.
Complication in open reduction was in 6 cases, such as non-union. 2 cases, infection. 2 cases, cross-union 1 case, and Volkmann¢¥s ischemic contracture 1 case.
Wringer injuries with open fracture of forearm bone (9 cases) are difficult to manage and Prognosis is poor with high rate of complication, such as non-union 2 cases, infection 2 cases, Volkmann¢¥s ischemic contracture 1 case, and original
injury, with nerve injury 5 cases, ligament injury 1 case. Author have not explained about functional result because of unsatisfactory follow tip study.
The Purposes of this article are to report 62 cases in fracture of forearm bo-nes, about initial management, and fixation method, especially treatment of the wringer injury and review of the literatures.
